Once you have registered for CALBREAK, there are a couple of forms that need to be completed and sent to us before we leave for California.  If you have not yet sent us these forms, please FOLLOW THE LINKS and send the completed forms no later than Friday March 31, 2025.  Please remember to have ALL required signatures before sending the forms to us. 

Parent Release Form must be NOTARIZED (in most cases both parents/legal guardians must be present in sign in front of the notary before they will notarize the form, so please plan accordingly).

If Calbreak Student will be 18 years old as of April 17, 2025, the parents/legal guardians do not need to sign the Parent/Legal Guardian Release/Waiver form or have it notarized.  However, in that instance only the student must sign the release/waiver form. 

We do require both the student and parent's/legal guardian’s signature on the rules form, as it is a community contract.
